What is the document for?
This document is needed for deregistration at the place of previous residence due to relocation. If the act of departure has been lost, the citizen must contact the Ministry of Internal Affairs at the new place of residence with a written application and detail all the circumstances.
The absence of a departure certificate is not an obstacle to filing an application for a new place of residence.
Russians are also deregistered:
- at the hotel;
- in the clinic;
- at a tourist base;
- in any place not intended for long-term residence.
If your stay was short, you do not need to declare deregistration. This only needs to be done if the citizen wants to leave earlier than planned.
When a person is discharged from the hospital, the departure certificate is issued by authorized hospital employees.
How to use when changing registration
In the practice of registering internal movements of citizens and migrants, there are two options for deregistration at the place of residence. Both are regulated by the Administrative Regulations of the Ministry of Internal Affairs. According to it:
- a citizen has the right, before leaving his previous place of residence, to report this in writing to the territorial department of internal migration of the Ministry of Internal Affairs. A mark (stamp) will be made in the passport and an address sheet will be issued;
- a citizen has the right to move to his next place of residence without deregistration. It will be “issued” upon request during registration.
Combining these procedures will not affect the timing of the new registration. The registration mark will be given within the same period - within 3 days.
IMPORTANT!
The provisions of regulatory legal acts on the possibility of deregistration “upon request” are often illegally ignored by employees of the migration division of the Ministry of Internal Affairs. This is due to the extra work involved.
What does a leaf look like?
The form of the document was regulated by Russian legislation. It must be filled in on both sides. The form contains a machine-readable data block that does not need to be filled out.
Citizens of the Republic of Belarus who wish to live and work in Russia need a Form 20 departure slip.
In Minsk and regional centers, this document is drawn up in 3 copies. In other settlements - in 2 copies.
After marking the departure, one copy is returned to the applicant along with an identification .
How to fill out a certificate using the ESM-7 form
First you need to provide the following input data:
- Help number.
- The date of its compilation.
- Name, address, telephone number, OKPO code of the performing company.
- Name, address, telephone number, OKPO code of the customer company.
- The name of the facility where the work was carried out and its address.
- The name and make of the car, its state license plate.
- Full name of the drivers who worked on it.
- Operation type code and work period.
The table below provides information about the work performed and the payment for it:
- The name of the work and its code.
- How many machine hours were worked?
- The cost of one machine hour and the work performed in general.
Then they summarize the results at the end of the table, note whether there were any downtime due to the fault of the customer, what the total amount for the work is, including VAT, and the VAT itself.
After this, you must indicate in words how many machine hours were worked. And at the end, the customer and the contractor sign.
How to fill out the form
It is advisable to fill out the form on a computer. This is much faster and more convenient for both the applicant and authorized employees. But you can print the form and fill it out by hand.

There are 15 questions on the departure sheet. On side “A” there are points from 1 to 11. Answering them is mandatory.
On side “B” there are points 12 to 17. Point 12 is mandatory. Not everyone needs to answer the remaining questions.

Procedure for marking departure:
- Contact a department of the Ministry of Internal Affairs.
- Fill out form 7.
- After filling out the departure address sheet in Form 7, the authorized employee puts the date and certifies the document with a personal signature. This means that he has no comments on the information and documents presented.
- After completing the procedure, the Russian citizen is returned with a stamp.
The mark is affixed by the person who is responsible for receiving and transmitting documents to the registration authorities.
About the ESM-7 form
This form was introduced by Decree of the State Statistics Committee of Russia dated November 28, 1997 No. 78. It is unified and was mandatory for use until the end of 2012. At the beginning of 2013, all forms became only recommended for use, and ESM-7 is no exception. However, although companies have the right to work with their own forms, some prefer the legally approved option. Such forms are familiar to employees of inspection agencies and contain all the necessary data.
If the organization decides to use its own developed forms, then it is necessary to include all the required details in them. This is the name of the company, the name of the document, the date of its preparation, the essence of the fact of economic life, the positions of the responsible persons, all the necessary signatures, units of measurement (according to paragraph 2 of Article 9 of Federal Law No. 402-FZ “On Accounting”).
What to do if you lose a leaf
If a Russian has lost his departure certificate, Order of the Ministry of Internal Affairs of the Russian Federation of October 23, 1995 No. 393 (Extracts) applies to him.
The procedure for restoring a sheet:
- Contact your department of the Ministry of Internal Affairs with a statement.
- Fill out a new form in two copies.
- Submit a new sheet, be sure to clarify that it needs to be restored if lost.
A new sheet is issued to the applicant within a week. It is certified by the seal of the Ministry of Internal Affairs.
Getting help
You can receive this document at the place of departure, and if it is lost, at the new registration address, after completing the appropriate procedure (written application, requests with confirmation at the former place of registration).
To do this, you must contact in writing the person responsible for submitting information to the relevant authorities of the Ministry of Internal Affairs, where registration is carried out. At the passport office - submit the relevant documents:
- written statement with a personal signature (free form);
- passport;
- information on payment of the fee (state duty), payment details can be found in advance on the website of the regional passport office.
Departure may be forced (criminal liability, eviction), then the FMS representative will issue this certificate himself. Likewise, upon arrival, registration is carried out without consent.

Questions and answers
Will they accept my documents for my new place of residence without Form 7?
Yes, they will. Previously, without this document, you could have been denied registration. But now the Regulations have been changed in your favor: documents are accepted, but the request is sent to your previous place of residence. The employee will provide you with a written explanation.
What if I lost both my departure slip and my passport?
In this case, a duplicate will not be issued. You need to apply for a new passport.

Form 7-injury: form and filling it out
The report is drawn up in accordance with the Instructions for completion given in the same Order dated June 21, 2017. No. 417. The reporting systematizes quantitative data on recorded accidents in the workplace. If an enterprise has separate divisions that independently pay wages to their personnel, statistical reporting for them is submitted separately, and information on separate structures should not appear in the head office form.
All data reflected in the report must have documentary evidence. Thus, the number of victims must coincide with the value shown in the report on form N-1 (about an industrial accident). When designating cost parameters, one must rely on data from accounting registers. If the injury report (Form 7) contains data on at least one victim at the end of the year, in the application generated every three years, this employee must be correlated with one of the causes of the incident.

How to fill out the annual form 7-injury:
- on the first page the reporting period is entered (for example, 2019);
- The full name of the legal entity is entered, indicating the abbreviated version of the name in brackets;
- the address must contain the name of the region, locality, street (alley, etc.), number of the house, building, office, zip code - if the legal address differs from the actual (postal) address, you must enter the data of the actual location of the company;
- in the block with codes, you must enter the OKPO of the legal entity; if we are talking about a separate division or branch, you must display its identification number;
- on the second page, form 7-injuries 2021 contains data on the number of victims and the amounts spent on occupational safety measures.
Let's look at filling out the form using an example.
At the enterprise over the past year, according to the act, there were three victims of accidents (two men and one woman), in addition, the development of an occupational disease was established in one employee of an occupational disease.
Line 01 indicates the total number of victims of incidents at production facilities - 3 people.
In lines 02-04, you need to detail the categories of victims, that is, whether there were women among them (in our case - 1 person), minors or foreigners. Similar detail is provided for fatal accidents (columns 05-08), with all these victims already included in the total number in line 01.
In line 09 you must enter the total number of days of incapacity for work of the victims according to their sick leave. If an accident occurred in the reporting period, and the certificate of incapacity for work is closed in the next year, as a unit the injured employee is included in the number in column 01, and the days of his incapacity for work will be included in the report of the next year.
If there were cases of transfer of victims to other positions with a lighter work schedule on medical advice, the number of such workers is shown in line 10. Cases of occupational diseases are indicated in field 12.
The company's annual expenses for implementing labor protection measures (from all sources of financing) are entered in line 13 in thousands of rubles.
At the bottom of the report, it is necessary to reflect the average number of personnel at the enterprise and highlight the number of women employed in the company, excluding maternity workers (fields 14 and 15). The number includes external part-time workers; the data must correspond to labor reporting. Workers under GPC contracts are not taken into account.
The reporting business entity must show whether there is a medical center (medical unit, etc.) on the territory of the enterprise or not; the required option should be circled. If there is a doctor on staff who provides emergency care, this is considered to have a health center.
The final stage is affixing the signature of the person responsible for filling out the form and indicating a phone number and email for communication.
Mail help
You can contact the previously listed services by mail. This is not the best solution - it takes a lot of time, money and effort. But in practice, such a situation still occurs.
Would you like to order form 7 or 9 by mail? Then it is recommended:
- Make copies of documents prepared in advance and then have them certified by a notary.
- Fill out an application for issuing a certificate of one type or another.
- Send an application with copies of documents to the authorized body. It is better to immediately send a letter to the local migration service.
- Get your hands on the standard form. Usually it is also sent to the customer by post.
Not too quickly, but sometimes this is the situation that helps to get certificates for housing.
When documents are required
Where can I get certificates forms 7 and 9? First, it’s worth understanding under what circumstances such documents may be useful.
They are usually requested if:
- it is necessary to formalize a legal transaction with property;
- there is a need to confirm registration or characteristics of the property;
- redevelopment is planned and legalized;
- There are plans for litigation related to housing.
Various categories of citizens can request such extracts. True, third parties cannot issue the appropriate forms. This is simply not provided for by current legislation.
To whom is it issued?
Where can I get certificates forms 7 and 9? It is worth paying attention to the fact that not every citizen can request the relevant documents. And therefore, in the process of achieving the desired goal, problems are not excluded.
At the moment, the above certificates can be issued by:
- residents of an apartment or house;
- owner of the premises;
- heirs;
- government agencies;
- representatives of the above persons.
Accordingly, it will not be possible to complete the previously mentioned forms without an official power of attorney. Third parties simply do not have such a right.
Certificates of characteristics of residential premises
Clause 47 of the Order of the Government of the Russian Federation dated April 25, 2011 No. 729-r “On approval of the list of services provided by state and municipal institutions and other organizations that place a state task (order) or a municipal task (order), subject to inclusion in the registers of state or municipal services and provided in electronic form” stipulates that the issuance of a copy of a financial and personal account, an extract from the house register, certificates and other documents in the field of housing and communal services falls within the powers of the relevant government agency of the constituent entity of the Russian Federation.
In accordance with clause 50 of the Consolidated List of Priority State and Municipal Services Provided Electronically, approved by Order of the Government of the Russian Federation dated December 17, 2009, 1993, the issuance of documents (a single housing document, copies of financial and personal accounts, extracts from the house register , registration cards of the owner of residential premises, certificates and other documents) are carried out by state and municipal institutions in the field of housing and communal services.
The body whose powers include issuing certificates of characteristics of residential premises and other similar certificates is determined by the subject of the Russian Federation. In St. Petersburg, for example, such a body is the GKU ZHA (Housing Agencies of the Districts of St. Petersburg).
Ways to apply for statements
How do I request the forms mentioned earlier? There are different ways to cope with this task.

- seek help from official representatives.
At the same time, everyone chooses for themselves how to act. It is best to use the first scenario. It is the fastest and most reliable.

Certificate Form 9 is a document that contains information about all persons registered in a residential premises who have the right to use this residential premises.
The Form 9 certificate indicates the full name of the owner of the property, then lists each person registered in this premises, their relationship with the owner of the property, date of birth, date of registration and type of registration of each of the people registered in this premises at the moment.
Certificate in Form 9 is a document necessary for the buyer; the information contained in it shows whether or not a citizen registered in a particular residential premises took part in privatization. A person registered but who did not take part in privatization, by law, retains a lifetime right to use this residential premises.

The certificate in Form 12 contains information about the deregistration of certain categories of persons who have the right to subsequently apply for residential premises. Unified form T-7: is it necessary to use it?
For the vacation schedule, a unified form T-7 , approved by Decree of the State Statistics Committee of January 5, 2004 No. 1. However, employers are not required to use it unchanged. Based on the resolution of the State Committee of the Russian Federation on Statistics dated March 24, 1999 No. 20, the form can be modified. You can also develop your own template, approve it and use it as a vacation schedule.
In practice, employers most often use a unified form, although the table is often supplemented with the necessary columns.

Algorithm for registering their living quarters
When leaving a residential premises, each citizen is obliged to record this fact with the migration registration authorities. The legislation provides for a certain algorithm of actions in this case.

What is a departure certificate, as well as other nuances of registration are regulated by separate legislative acts. The main one is Decree of the Government of the Russian Federation No. 713 dated (latest edition from) “On approval of the Rules for registration and deregistration of citizens of the Russian Federation at the place of stay and place of residence within the Russian Federation and the list of persons responsible for receiving and transferring documents to registration authorities for registration and deregistration of citizens of the Russian Federation at the place of stay and place of residence within the Russian Federation.”
Lost departure slip - what to do
Moving is a troublesome business. And in the turmoil of collecting things and organizing other things, documents may remain in the old apartment. Citizens who are faced with similar unpleasant situations are wondering what to do if the departure slip is lost.
Considering the fact that when registering, it is not necessary to have this form of document with you, the answer is simple: you don’t need to do anything. You just need to fill out a request for deregistration at your previous address in the application for a new registration.
What is needed to obtain a departure certificate
Having understood the peculiarities of filling out a departure form, it is logical to discuss the algorithm of actions for citizens to complete it. To confirm the legal fact of the extract, you need to contact the relevant government authorities with a certain package of documents.
What documents need to be provided
For those who are interested in how to obtain a sheet of form No. 7, the following list of documents is provided:
- an identity card, because almost all the data on the departure form is filled out on its basis: for citizens of the Russian Federation - a passport;
- if a child is being discharged, a birth certificate;
- for a temporary resident - a residence permit;
- for a foreigner - a passport of his home country;
The forms for minors include the arrival and departure addresses of their parents.
Authorized bodies
To obtain a departure certificate, a citizen needs to contact the relevant government agencies. In the past, this was done by passport offices. Currently, issues of registration of citizens are under the jurisdiction of the territorial branches of the Directorate for Migration Issues of the Ministry of Internal Affairs of the Russian Federation.
In addition, migrants who are puzzled by the question of where to obtain a departure certificate can contact the nearest Multifunctional Center for the Provision of Public Services.
Price and processing time
The deadline for issuing a departure certificate is no more than 3 days. The applicant is returned one copy of completed form No. 7 and a passport with a note about the extract.
The procedure for deregistration is free; administrative regulations do not provide for a state fee for issuing a departure certificate and affixing a deregistration stamp in the passport.
Which form to use for the 2021 report
Organizations that reported earlier have the right to use the old form; officials have not yet introduced the new 7-injury report form for 2021. The document itself was approved by Rosstat order No. 417 dated June 21, 2017.
If officials do not change anything, and the 7-injury report form for 2020 remains, the same form will be allowed to be used for reporting for the current year.
What is not included in the vacation schedule
The schedule does not include vacations:
- Subject to compensation as unused. For example, if the duration of the employment contract is short, and in principle it does not provide for vacation. Or - if the employee’s next vacation is due (while the employer and employee have not agreed otherwise) in a year for which a schedule has not yet been drawn up, and the employee quits before the beginning of this year.
- Educational.
The schedule should reflect only basic and additional leaves, including extended ones, which are established for various categories of workers - for example, teachers.
Making a schedule is half the battle. It is important to maintain its current status, including by timely correction (and addition) of the information reflected in the document.
