Form 7-injuries is an annual report on accidents and occupational diseases that occurred at the enterprise. Use step-by-step instructions for completing and submitting a report.

Form 7t (injuries) for 2021) are submitted to the territorial division of Rosstat at the location of the organization or individual entrepreneur once a year - no later than January 25 of the year following the reporting one.

But this report is not required for everyone. It does not need to be prepared for micro-enterprises, households, as well as:

  • financial and insurance companies;
  • institutions involved in public administration and military security;
  • educational organizations;
  • social security institutions;
  • extraterritorial organizations.

Who and why is taking form 7-injury in 2021

Using form 7-injuries, the state statistics service collects information on how many injuries and diseases occurred in Russian industries in 2021.

Legal entities of any form of ownership and types of activity are required to report on this form. But there are a number of companies that may not transmit information about employee injuries and illnesses to the statistics service. The following are excluded:

  • micro-enterprises - less than 15 employees and 120 million in revenue;
  • banks and insurance companies;
  • organizations in the field of government or national defense;
  • educational organizations;
  • households;
  • international companies and representatives of companies from other countries located in the Russian Federation.

To whom and when to submit a report on injuries to statistics

Report 7-injuries is submitted by legal entities, except for micro-enterprises, with the exception of those who conduct such types of economic activities as:

  • financial and insurance;
  • public administration and military security;
  • social Security;
  • educational;
  • household activities;
  • activities of extraterritorial organizations.

Organizations that meet the above criteria and do not belong to small businesses are required to submit a statistical report on injuries, and small enterprises - only if they are included in the sample of statistical authorities. This is stated on the Rosstat website.

Territorial branches of Rosstat must inform organizations whether the enterprise is included in the sample. At the same time, clause 4 of the regulation “On the conditions for the provision of statistical data” (approved by Decree of the Government of the Russian Federation dated August 18, 2008 No. 620) states that information is provided free of charge, including in writing. But Rosstat’s letter No. 04-04-4/29-SMI dated February 17, 2017 recommends checking information about what forms of statistical reports an organization should submit on the Rosstat Internet portal using OKPO, INN or OGRN codes.

Provide a statistical report on injuries annually, before January 25 of the year following the reporting year. You must submit the report for 2021 no later than January 27, 2020 (postponed from Saturday, January 25).

Penalty for initial failure to provide statistical data in accordance with Art. 13.19 of the Code of Administrative Offenses is:

  • for officials - 10,000–20,000 rubles;
  • for legal entities - 20,000–70,000 rubles.

And for the subsequent ones:

  • for officials - 30,000–50,000 rubles;
  • for legal entities - 100,000–150,000 rubles.

But fines cannot be applied if information about the need to submit statistical reporting was not on the Rosstat website, and the organization was not informed in writing. Screenshots of Internet pages can serve as proof that a legal entity is right.

Legal basis of form No. 7-injury, changes 2021

The form is designated as 7-injury, and as management documentation it is assigned the code 0609304. The Statistics Service collects information on how many injuries and illnesses occurred in Russian industries in 2021. The procedure for filling out the report and the current form were approved by Rosstat Order No. 417 dated June 21, 2017.

Since the beginning of 2021, the worker injury and illness report has not changed at all. However, it is worth remembering that along with the main form 7-injury, you must take the application every 3 years. It contains data on accidents that have occurred by type and reveals their causes. The frequency of delivery is fixed at the level of law; it is not related to the operating time of the business entity. In 2020, Rosstat expects a main report with an appendix.

Who delivers and when?

Like the main report 7-injuries for 2021, an appendix to it is prepared by enterprises with more than 15 employees, except:

  • providing services in the field of finance and insurance;
  • institutions from the sphere of public administration;
  • enterprises involved in ensuring military security;
  • educational institutions;
  • households;
  • extraterritorial organizations.

The document is submitted to the territorial division of Rosstat once every 3 years no later than January 25 of the year following the reporting year. You should report for 2021, 2022, etc. That is, the supplement to form 7-injuries for 2021 will have to be prepared in 2021 by those who reported in 2021.

Filling out form 7 injuries: report structure and detailed instructions

Form 7-injury consists of a title page, a large table of indicators and a reference part. Let's look at each component in more detail and give step-by-step instructions.

Title page

The title page should include information that is standard for all statistical reports. To begin, indicate the year for which you are submitting the report - 2021.

Enter the name of the organization in the appropriate line, as it is given in the registration documents, and indicate the short name in brackets.

In the “mailing address” line, enter the name of the subject of the Russian Federation, city, street and house, do not forget to indicate the postal code that corresponds to the legal address. If you actually work in another location, then indicate the current address where you operate.

Tabular part

Information about work-related injuries and injured employees should be taken from reports in Form N-1, which are drawn up for each accident. Data on the organization of security measures and the costs of them can be taken from accounting.

The entire table contains 13 rows. For each line, you must indicate the requested indicator for 2021 in the units of measurement specified in the third column. Let's look at the lines with which difficulties most often arise.

Line 01 is the result line. It must indicate the number of employees injured at work if the accidents resulted in the inability to work for 1 working day or more or resulted in death.

This line does not need to take into account deaths in the reporting year that occurred as a result of accidents that occurred in the years preceding the reporting year. The same applies to employees whose temporary disability began in 2021 and ended in 2019.

In lines 02 - 04, it is necessary to distribute the number of people from lines 01 into categories: women, children and foreigners.

On line 05, indicate the number of deaths due to industrial accidents during 2021. The time of occurrence of the accident does not matter if the death occurred in the reporting year. In lines 06 - 08, also categorize them as women, minors and foreign citizens.

On line 09, enter how many days during the year all employees remained unable to work. To do this, you need to sum up the number of all days of incapacity for work as a result of accidents at the organization's production site. Take information from medical reports for employees whose disability ended in 2019. If it is still ongoing, then it can be ignored.

In line 10, highlight the number of victims who partially lost their ability to work and, in accordance with the medical report, were transferred to easier work for a period of 1 working day. In line 11, highlight the women. These lines do not need to take into account employees who received sick leave.

In line 12, enter the number of employees who were diagnosed with a disease related to their profession for the first time in 2019. An employee can be included in the line on the basis of a medical report from a specialized organization and an internal document (directive, order).

If labor protection measures were carried out during the year, then indicate the amount of expenses for them in line 13. The source of the funds does not matter.

Help part

In the reference section, indicate the three requested indicators. On line 14 - the average number of employees, on line 15 - the average number of female employees. These include full-time employees and external part-time workers. Women on maternity leave are not counted.

In line 16 it is necessary to put o if the company has a first aid station or doctors who are able to provide first aid to victims.

How to fill

Similar to the report, the attachment to form 7-injury consists of two parts (title page and table) and is filled out on the basis of an act in form N-1. Let's look at the basic rules for preparing an application.

Title page

Filling procedure:

  • You must first indicate the reporting period for which the document is being submitted - 2021;
  • Enter the full name of the organization. In parentheses we indicate the abbreviated name of the enterprise (if available);
  • We provide the full postal address of the organization (with the obligatory indication of the postal code).

Tabular part

On the second sheet we distribute the number of affected citizens according to the indicators in the table. Please note that when filling out information, fatal accidents should be highlighted in column No. 4:

  • line 01 should contain information about the number of all victims;
  • in lines 02 to 15, the number of injured employees should be distributed by type of incident;
  • lines 16-30 classify accidents according to their causes.

Form and place of submission of report 7-injuries

If the company has separate divisions, then for each of them you need to fill out and submit a separate form. The additional form is submitted for a legal entity without taking into account its divisions.

It is necessary to report to the territorial body of the FSGS, which corresponds to the legal address of the legal entity or its division. In cases where the place of registration and place of business do not coincide, the form is submitted to the authorities at the place of business.

The form can traditionally be submitted on paper or electronically.

General filling requirements

Traditionally, Rosstat offers to fill out a paper or electronic version of the report. If you submit information on paper, then any corrections or erasures are prohibited. Handwriting must be legible and only use black or blue ink.

The necessary information about cases of injury at work is taken from accident reports (Form N-1, approved and filled out in accordance with Resolution of the Ministry of Labor dated October 24, 2002 No. 73). Amounts spent on labor protection are reported using accounting data. If there is no data, enter 0.

Responsibility for failure to submit a report

Organizations that failed to submit a report on time or included false information in it will be subject to economic sanctions under Article 13.19 of the Administrative Code. There are fines for these violations.

How to fill out

The 7-injury report is divided into four stages:

  • title page;
  • table;
  • certificate form 7-injury;

Title page

Industrial injuries: what is it, types

Here you need to write information about the organization, address and date. Also:

  • current year of filling;
  • full name of the company followed by a short one. Be sure to put the short one in brackets. For example, Kazan Fat Plant LLC (KZhK LLC);
  • company address. Here you need information on the actual address of the enterprise;
  • OKPO code. You can find it through the “Contour.Focus” service. You can enter your TIN on the website, and the system will show the code.

Important! If there is a branch, then the OKPO code will be different. In this case, you will need to additionally find out the branch code.

Table

Here you need to fill in all accidents, injuries acquired by the employees of the prof. diseases for the whole year. Opposite each item you must put a numerical value:

  • how many people were injured at work. If an employee was injured and took sick leave for a day or more, then it must be taken into account.
  • how many of them were women, minors and foreigners;
  • number of deaths;
  • of which women, foreigners and minors;
  • total number of sick days for the reporting period;
  • if the sick leave was not opened, but the employee was transferred to another activity for medical reasons, then it must also be entered here;
  • transferred women for the same reasons;
  • the amount allocated for labor protection measures. The company's accountant will provide this information.

Important! If the death occurred during the reporting period, and the person was injured earlier, then it does not need to be indicated in the first line. Same with point 9. If a person opened a sick leave earlier than the reporting period, then the information is written only in line 9.

Help 7-injury

Here you need to indicate the number of employees in the organization. This is necessary in order to determine the extent of the injury. You should also indicate whether the company has its own medical center.

In the contact information column, you must put a date and sign, and leave contact details.
