Why do you need Form 11 for 2021 in 2021?
Using Form 11, all legal entities, except those related to SMEs and non-profit organizations, must annually report to Rosstat (before April 1 of the year following the reporting year) on the availability and movement of fixed assets and other non-financial assets (NFAs) used in business.
Reports for 2021 must be submitted before 04/01/2021 using the new Form 11, approved by Rosstat Order No. 384 dated 07/15/2020. Its form is available for download on our website below. Instructions for filling out form 11 were approved by the same Rosstat order No. 384.

We report on injuries and occupational diseases at work using a new form! Updated
Greetings, friends! Managers and specialists, be aware that information on workplace injuries and occupational diseases from the 2021 report must be submitted using a new form (Federal Statistical Observation Form No. 7-Injuries). We remind you that the reporting is annual, the deadline for submission is January 25. You will also find out who to submit the report on Form 7 from this note.
The new injury form was approved by Rosstat Order No. 326 dated June 22, 2020 “On approval of the federal statistical observation form for organizing federal statistical observation of industrial injuries and occupational diseases.” You can download Rosstat order No. 326, as well as Form 7 injuries in MS-Excel format just below.
With the introduction of a new injury form, Rosstat Order No. 417 of June 21, 2021 “On approval of statistical tools for organizing federal statistical monitoring of industrial injuries and occupational diseases” is invalidated.
Rosstat Order No. 326 also provides instructions for filling out Form 7 injuries. The content of the instructions is not large, so we will present the text in its entirety.
Instructions for filling out the federal statistical observation form
1. Federal statistical observation form No. 7-injuries “Information on injuries at work and occupational diseases” (hereinafter referred to as the form) is provided by legal entities (except micro-enterprises) engaged in all types of economic activities, except: financial and insurance activities, activities in operations with real estate, public administration and military security, social security, education, activities of households as employers, undifferentiated activities of private households in the production of goods and provision of services for their own consumption, activities of extraterritorial organizations and bodies.
How to find out the list of reports to Rosstat?
To check the list of reports for submission to Rosstat, follow these steps:
Copy your organization's Taxpayer Identification Number.
Go to the Rosstat website.
Insert your Taxpayer Identification Number (TIN) into the appropriate field.
Click “Get” > the reports that need to be submitted will be downloaded.
The form is provided by legal entities of all forms of ownership.
When compiling the form, completeness of completion and reliability of the primary statistical data contained in it (hereinafter referred to as the data) must be ensured.
The “For reference” section must be completed without fail.
2. If a legal entity has separate divisions <1> this form is filled out both for each separate division and for a legal entity without these separate divisions.
<1> A separate division of an organization is any division territorially isolated from it, at the location of which stationary workplaces are equipped. Recognition of a separate division of an organization as such is carried out regardless of whether its creation is reflected or not reflected in the constituent or other organizational and administrative documents of the organization, and on the powers vested in the specified division. In this case, a workplace is considered stationary if it is created for a period of more than one month (clause 2, article 11 of the Tax Code of the Russian Federation).
The completed forms are submitted by the legal entity to the territorial bodies of Rosstat at the location of the corresponding separate division (for a separate division) and at the location of the legal entity (without separate divisions). In the event that a legal entity (its separate division) does not carry out activities at its location, the form is provided at the place where it actually carries out activities.
If a legal entity has separate divisions located on the same territory of a constituent entity of the Russian Federation as the legal entity, the data in the form is provided for the legal entity as a whole, including data on separate divisions.
If a legal entity has separate divisions located on the territory of different constituent entities of the Russian Federation, data is provided for each separate division at their location.
In this case, it is possible to provide a consolidated report for all separate divisions of a legal entity operating in a specific subject of the Russian Federation, provided that the head of the legal entity appoints an official responsible for reflecting aggregated data for these divisions. In this case, the provision of the report is assigned to one of the divisions identified in a given subject of the Russian Federation.
If a legal entity has separate divisions operating outside the Russian Federation, data on them is not included in this form.
If the organization did not conduct any activity during the reporting period, then it submits the form with completed lines 23 - 25.
The head of a legal entity appoints officials authorized to provide data on behalf of the legal entity.
The form is also provided by branches, representative offices and divisions of foreign organizations operating on the territory of the Russian Federation in the manner established for legal entities.

The address part indicates the full name of the reporting organization in accordance with the constituent documents registered in the prescribed manner, and then the short name in brackets. On the form containing data on a separate division of a legal entity, the name of the separate division and the legal entity to which it belongs is indicated (for example: Branch No. 19 of Krasny Tekstilshchik JSC).
The line “Postal address” indicates the name of the subject of the Russian Federation, legal address with postal code; if the actual address does not coincide with the legal address, then the actual location of the respondent (postal address) is indicated.
In the code part of the title page of the form, based on the Notification of assignment of the OKPO code (identification number), posted on the Rosstat website at: https://websbor.gks.ru/online/#!/gs/statistic-codes, the reporting organization puts down:
- code according to the All-Russian Classifier of Enterprises and Organizations (OKPO) - for a legal entity that does not have territorially separate divisions;
- identification number - for a territorially separate division of a legal entity and for the head division of a legal entity.
The head unit of a legal entity is a separate unit where the administration of the enterprise is located or whose location corresponds to the registered legal address.
Bankrupt organizations that have entered bankruptcy proceedings are not exempt from providing data on the form. Only after the arbitration court has issued a ruling on the completion of bankruptcy proceedings and entry into the unified state register of legal entities of its liquidation (clause 3 of Article 149 of the Federal Law of October 26, 2002 No. 127-FZ “On Insolvency (Bankruptcy)”) The debtor organization is considered liquidated and is exempt from providing data on the form.
When reorganizing a legal entity in the form of transformation, the legal entity that is the legal successor, from the moment of its creation, must provide a report in the form (including data of the reorganized legal entity) within the period specified on the form for the period from the beginning of the reporting year in which the reorganization took place.
3. The form reflects data on victims of industrial accidents in accordance with the Act on an industrial accident in form N-1 or the Act on an industrial accident in form N-1PS (hereinafter referred to as the act of form N-1 and the act of form N-1PS, respectively), approved by Resolution of the Ministry of Labor of Russia dated October 24, 2002 No. 73 “On approval of the forms of documents necessary for the investigation and recording of industrial accidents, and provisions on the peculiarities of the investigation of industrial accidents in certain industries and organizations” ( registered by the Ministry of Justice of Russia on December 5, 2002, registration No. 3999), articles 227 - 231 of the Labor Code of the Russian Federation.
4. Data on the costs of labor protection measures are shown according to accounting data.
5. Line 01 shows the number of victims with loss of ability to work for 1 working day or more, including fatalities. On lines 02 - 04 - respectively, the number of affected women, persons under 18 years of age, and foreign citizens. On line 05 - the number of victims for whom in paragraph 10 of the act of form N-1 (in paragraph 9 of the act of form N-1PS) employees of the reporting organization are indicated as the perpetrators. On line 06 - the number of victims for whom in paragraph 8.3 of the act of form N-1 (in paragraph 7.1 of the act of form N-1PS) information is indicated about the victim being under the influence of alcohol or drugs.
6. Line 07 reflects the number of victims whose death occurred in the reporting year, regardless of the time of the accident. On lines 08 - 10 - respectively, the number of affected women, persons under the age of 18, and foreign citizens. On line 11 - the number of victims for whom in paragraph 10 of the act of form N-1 (in paragraph 9 of the act of form N-1PS) employees of the reporting organization are indicated as the perpetrators. On line 12 - the number of victims for whom in paragraph 8.3 of the act of form N-1 (in paragraph 7.1 of the act of form N-1PS) information is indicated about the victim being under the influence of alcohol or drugs.
If a death occurs in the reporting year after an accident that occurred in the year preceding the reporting year, this case is not reflected on line 01 of this report, since it should already be taken into account on this line in the report for the previous year.
7. Line 13 reflects the number of working person-days of disability for victims with loss of ability to work for 1 working day or more, whose temporary disability ended in the reporting year. The number of days of temporary disability is reflected in total for all certificates of incapacity for work issued by medical organizations. In the case where the victim was injured in the year preceding the reporting year, and his temporary disability ended in the reporting year, the total number of person-days of disability is shown on line 13 in the report for the reporting year. This case is not reflected in line 01, as it was already taken into account in the report for the previous year.
8. Line 14 shows the number of victims who partially lost their ability to work and were transferred from their main job to another for 1 working day or more in accordance with a medical report, but without issuing a certificate of incapacity for work. If a certificate of incapacity for work was issued to the victim, then this line is not filled in. Line 15 reflects the number of women who have partially lost their ability to work.
9. Line 16 shows the number of persons with an occupational disease diagnosed for the first time in the reporting year on the basis of a conclusion issued by the relevant treatment and prevention organization and issued by an internal order for the organization (order, etc.).
If there is no case of occupational injury in the reporting year, lines 17, 23 - 25 are filled in.
10. Line 17 reflects the costs of labor protection measures, including the costs of improving conditions and labor protection at work, at the expense of all sources of financing in accordance with the collective agreement and the labor protection action plan (order of the Ministry of Health and Social Development of Russia dated March 1, 2012 No. 181n “On approval of a standard list of measures annually implemented by the employer to improve working conditions and safety and reduce levels of professional risks” (registered by the Ministry of Justice of Russia on March 19, 2012, registration No. 23513) (hereinafter referred to as the Standard List). Line 18 is reflected costs for the purchase of work clothes, special footwear and other personal protective equipment (item 19 of the Model List). Line 19 reflects the costs of implementing organizational measures (items 1, 2, 25 - 28, 30, 32 of the Model List). Line 20 reflects the costs of implementation of technical and technological measures (items 3 - 12 of the Model List) Line 21 reflects the costs of implementing sanitary and hygienic measures (items 13 - 18, 20 of the Model List). Line 22 reflects the costs of training workers in labor protection (items 21 - 24, 29, 31 of the Model List).
Financing of labor protection measures is prescribed in Article 226 of the Labor Code of the Russian Federation.
11. On line 23 the average number of employees is entered, consisting of payroll employees and external part-time workers, based on labor reporting.
12. On line 24 the average number of working women is entered, consisting of payroll employees and external part-time workers (excluding women on maternity leave and additional child care leave). Employees who have entered into a civil contract with the reporting organization are not included in lines 23, 24.
13. Line 25 is filled out by institutions, organizations (including medical and preventive organizations) that have on their staff doctors, paramedics, nurses who can provide primary emergency care, noting this as the presence of a health center (doctor’s office, medical unit and the like).
