How to fill out a certificate of services rendered with an individual
The form of the act of services rendered does not have a single template, which is fixed by legislative standards. Therefore, persons drawing up such documentation can draw it up in free form. The document can be typed or handwritten.
There are no special features in filling out a certificate of services rendered with an individual, except that one of the parties to contractual obligations is a private individual.
If the act is drawn up by a legal entity, then the organization can use the form already established in its accounting policies.
The acceptance certificate for services provided is drawn up in two copies and handed over to both parties. For the documentation to have legal force, you need to include the following basic details:
- title of the paper, information about its preparation (date, place);
- information about which agreement the act is attached to;
- participants of the delivery and acceptance are registered;
- information is provided about the fact of economic activity (in this case, about the service). Its essence, when it was performed and by whom, what work was carried out under the contract;
- signatures are placed.
Important! The text must state that the customer has no complaints about the quality of the services.
If during acceptance and transfer it turns out that the services were not performed in full, do not meet the requirements of the customer’s assignment, or were performed poorly (defects or malfunctions were found), then such items must be described in detail in the act.

Design rules
The document can be drawn up on a regular A4 sheet or on the company’s letterhead, either by hand or on a computer (it doesn’t matter). You need to make two copies - one for each side. In this case, both copies of the act must be signed by the heads of both organizations (or employees authorized to act on their behalf) and stamped with seals (only if they are available, since from 2021 legal entities by law have the right not to endorse their papers using stamps and seals ).
After the service has been provided and cooperation has been completed, including full payment, the act should be transferred to the company’s archives, where it should be stored for the period required by law.
How to fill out a certificate of completed work for services
If we take a free-form document as a basis, then in fact it consists of three large blocks:
- 1st block of documentation - name, date of compilation, location, registration number, if applicable;
- 2nd block: information about the services provided. If there were several works, it is convenient to arrange this part in a tabular form;
- 3rd block: details of participants. For legal entities – names, organizational and legal forms, authorized representatives. For individuals – full name, passport details, residence or registration address.
At the end, the document is certified by the signatures of both parties.
If we take this form as a basis, then to fill out you will need:

After registering on the site, performers see orders for their type of service. You can select a suitable order based on budget, timing and location - and send a response to the client, in which you can briefly tell about yourself and set your own price range for the service.
What are the commissions?
Payment in the service depends on the type of activity of the performer. Some specialists pay for orders, others - for responses. There is no need to pay a percentage of the cost of orders - the commission amount is always individual and consists of several factors. You can see it before accepting your order.
Let's take a closer look.
Payment for orders
The service uses this commission for tutors, trainers, driving instructors, psychologists and other performers who usually provide their services repeatedly to the same clients. Such performers pay a fairly high commission - but only once per client. The amount is written off after the first paid lesson, and all money from subsequent lessons with clients remains with the performer.
The size of the commission depends on the rate at which the performer is willing to work with the client and the order parameters: for example, the subject, the client’s age, the purpose of the classes and the duration of the course.
Example.
Elena works as a tutor. A new student found her on Profi.ru and is ready to use her services. Elena must pay a commission: 1900 ₽. This commission will be debited from Elena’s account on the website after the date of the lesson, which Elena agrees on with the student. Elena can receive the student’s payment for this and subsequent classes on a personal bank card or in cash - the service does not charge a fee for paying for services.
Payment for responses
This commission is paid by the rest of the performers: everyone who provides a one-time rather than permanent service. For example, hairdressers, repairmen, couriers, event hosts, and so on. For such performers there is a price for response - offering their services to the client. The amount is debited from the account as soon as the contractor responds. Important: the client may not choose this artist, but the money will not be returned.
The size of the commission depends on the value of the order and its popularity. For example, the commission will be higher if the client is the first to offer you to perform a service - rather than you responding to his order.
Example.
Andrey is a plumber. He found a suitable order on Profi.ru. To respond to this order, Andrey paid the service 50 ₽. The client received five more responses, but he chose Andrey’s proposal. Andrey will receive payment from the client on a card or in cash - the service will no longer take a commission from this money.
Another example.
Irina is a manicurist. She found a suitable order on Profi.ru and responded to it for 50 rubles. The client received five more responses, and he did not choose Irina’s proposal. But the money that Irina paid in the form of a commission will not be returned to her.
In some cases, the service still returns money for the response. This is possible if the client did not even review the contractor’s response within five days, or the contractor responded to the order when another specialist announced an agreement with the client, but the client has not yet confirmed this.

How to take payment
The client pays for orders directly to the contractor. So you can take money in cash, to a card account (for example, by phone number) or to a current account if you are an individual entrepreneur or a legal entity.
How to fill out a certificate of completion of work under a service agreement
Filling out a certificate of completed work for a service contract always occurs in conjunction with the signed agreement. Thus, the paper is an annex to the contract.
The act for the service agreement is filled out in the same way as described in the section above. The documentation does not have a uniform form, regardless of the type of paid services.
If a company uses its own approved sample in its activities, the document must be approved in the company’s accounting policy.
How to develop a template yourself
Each customer organization has the right to independently decide what the work completion certificate should look like, develop a form and approve it in its local documents. When preparing it, provide the following information:
- Document Number;
- number of the contract that is being closed;
- date of preparation and signing;
- name, quantity and cost of work performed;
- cost taking into account the applicable taxation system;
- details of the parties;
- signatures and seal.
A simple sample act on the provision of legal services
The act on the provision of legal services is drawn up as the final stage of the transaction between the contractor and the customer. The paper is drawn up after the actual execution of the service, and it does not matter which specific one. This may include consultations, drawing up claims, representation in court, etc.
Participants in the act are two parties: the customer and the performer. As a rule, it is the latter who draws up the act.
The paper certifies not only the absence of claims for the work performed, but also confirms the need for payments for its provision. Especially in cases where an advance payment has been made.
A simple sample is compiled as follows:
1. Write down the paper details:
- in the middle – the name;
- just below: on the one hand - the date of compilation, on the other - the place.
2. Indicate the agreement to which the act was drawn up. Agreement, its number, date of preparation.
3. Write down information about the participants in the transaction. Here you can fill in a similar sample agreement:
- organizational and legal forms and names;
- authorized representatives;
- on the basis of which documents they act.
4. Indicate the essence of the services performed.
5. Write down that the customer has no complaints, the work was completed on time and in full.
6. Be sure to include information about the final payment for services.
7. Sign the paper.

The main service of law firms is consulting. Use the filling template described above. Filling out the act does not differ depending on the type of work performed.
Act on the provision of accounting services
The act is signed between the parties: the provider of accounting services and the customer of such work. The paper can be handwritten or printed. You can use a free form or a set pattern.
In what form is it compiled?
In other cases, the act is drawn up in free form. It is most convenient to draw up a sample act of provision of services in Word.

If necessary, the organization has the right to supplement the unified form KS-2. All developed forms must be approved in the accounting policies and local regulations of the customer.

The legislation does not establish strict rules for creating your own form. But for it to have legal force, you need to include the following information:

To do this, the form includes relevant items reflecting the actual quality of the work, identified deficiencies, the amount of penalties and the amount under the contract calculated after deduction of fines and penalties.
Corrective act for services rendered, sample filling
If, during the execution of agreements, inconsistencies, errors, or any conditions have changed, an adjustment report is drawn up. Moreover, if the essential conditions change, an additional agreement must be attached to the contract, which specifies the same changed clauses.
The adjustment document is drawn up in a free style, indicating the required points:
- name and other details of the paper (number, date of preparation);
- to which agreement the act is drawn up (data of the additional agreement are indicated here);
- information about the parties involved is recorded;
- in the main block the changed conditions are indicated in detail;
- The signatures of the parties are placed as confirmation of agreement with the act.
If, during the course of the agreements, non-essential conditions have changed that will not affect the course of the transaction and are not reflected in any way in the companies’ documentation, an adjustment act need not be drawn up.

This document indicates that, on the basis of the contract (agreement), work was performed or services were provided in full and on time. It reflects the quantitative and qualitative characteristics of the contractor’s fulfillment of the terms of the concluded government contract.

They draw up an act of provision of services or work performed even if the customer refuses to accept the work, and when the work is not completed in full or not at all. In such situations, the current protocol must reflect the detected violations that prevent the government customer from accepting the results, and indicate the period during which they will have to be eliminated.
Documentation serves as essential evidence in legal proceedings with the contractor when controversial issues arise under a contract, the subject of which is intangible services, the result of which cannot be demonstrated, assessed or measured. In this case, the document indicates completion and reflects the completeness and timing of the work.
Documentation on the approval of results is also important for the customer organization when preparing interim and final accounting reports on the use of allocated budget funds. A document signed by both parties is the basis for payment to the contractor and confirms the intended use of budget allocations.
Reasoned refusal to sign the certificate of services rendered
If the customer has complaints about the execution of work, the certificate of completion of work is not signed, a reasoned refusal is drawn up for it. Such a document has legal force and can be used in court as evidence in disagreements between counterparties.
It is important to correctly draft such a refusal and describe in detail the claims that have arisen - otherwise the document will not have legal force.
There is no single template for such a paper, but the sample should definitely include:
- title, date of compilation and place;
- indicate previously accepted agreements, additional agreements (if they were signed);
- describe in detail the claims that have arisen and compare them with the terms of the contract;
- describe who accepted the work, whether an examination was carried out. If yes, then by whom and what are its results;
- if necessary, attach photographic material, evidence of the low quality of performance (or non-performance) of work;
- calculate the losses incurred, prove them with documents and detailed calculations;
- sign the waiver.
One copy remains with the customer, the second is sent to the contractor.
The certificate of work performed under the service contract can be classified as mandatory documentation. After all, the contract itself only confirms that the parties agreed and accepted the terms of execution, but the fact of execution itself is confirmed by the act. Compilation rules
In accordance with the norms of office work, the act must contain the following details:
- date and place of compilation;
- Title of the document;
- reference to the agreement (date and number), the annex to which is the act and the basis for its preparation;
- information about each of the parties to the contractual relationship for the performance of work;
- a tabular part listing for each item of work their name, brief description, unit of measurement, quantity, price per unit and cost;
- information that the work was completed in full and on time as specified in the contract, and that there were no complaints from the customer (or a list of deficiencies with deadlines for their elimination);
- signatures of the parties to the agreement and seal impressions (if any).
It is recommended to fill out the part of the table indicating the name of the work as accurately and carefully as possible. This information may be used in legal proceedings if necessary.
Depending on the complexity and volume of work performed by the contractor, the parties have the right to create a commission for their acceptance. It should include representatives of both sides.

The commission may include an expert from a third-party competent organization or a specialist from a state regulatory body. In this case, the act must be signed by all members of the commission and certified by the heads of the parties to the agreement.
As a rule, after completion of the work, the contractor draws up an act, signs it and sends it to the customer, unless a different procedure is provided for by the terms of the contract. If there are no objections to the quality and timing, the second party signs and returns the copy to the contractor.
If the customer evades signing the act, the contractor has the right to do this unilaterally, indicating the fact of evasion.

Complete your profile.
Try to fill out all the fields in your profile as much as possible: full first and last name, address, service categories. Go through verification and confirm your documents: passport, education, qualifications.
Post a photo.
Your face should be clearly visible in the photo. It’s good if it’s a neutral photo or something related to your type of activity. “Open eyes and a slight smile, a calm background, quality, clarity and relevance of the photo - all these are signs of the profile of a person you can trust,” advise the press service of Yandex.Services.
Tell us about yourself.
In simple, easy and competent language, tell us what you do and what services you can provide. Write about your education, qualifications, work experience - if they are related to your services. Tell us about your advantages, achievements and positive qualities: why the client should choose you over other performers. If you have a portfolio or screenshots with reviews from satisfied customers, please provide a link or attach a photo, if possible.
Ask for feedback.
Clients of services are not required to leave reviews about performers. Ask them about it yourself after finishing work. To maintain a high rating on any service, you need to consistently receive positive reviews.
Complete more tasks.
The more active your profile is, the more often customers will notice it. And, of course, the more tasks completed, the more reviews and ratings, and this is the main thing that clients pay attention to.
